Document Request

Movahedi Solutions is the operating name of 1000823315 ONTARIO INC., based in Mississauga, Ontario, Canada. This page states how to ask for a company document, what is public, what is provided on request, what requires an agreement, and what to expect when a document does not exist. Contact: mohammad@movahedi.ca.

How to make a request: five steps

  1. Step 1: Send an email naming the document. Send an email to mohammad@movahedi.ca and name the document you need. Include your name, your organisation if applicable, and the purpose of the request. A request that does not name a document cannot be matched to a document, so the document name is the one required detail in every request. You may also use the on-site form below. The form prepares an email to mohammad@movahedi.ca.
  2. Step 2: Public items are linked directly, with a date. Public documents are linked in the table below, each with its date. You can read the current version directly from the link. There is no separate request step for a public document.
  3. Step 3: On-request items are sent in the current version, with a date, where they exist. For an item listed as on request, the reply is sent by email from mohammad@movahedi.ca. The reply sends the current version of the document and states its date. If the document has no date, the reply states that it is the current version in use at the time of the reply.
  4. Step 4: Where access requires an agreement, the requirement and the route are stated. Some material is available only under a signed agreement. In that case, the reply states the agreement required and the route to obtain it. The requirement is not left silent, and a request is not refused without an explanation of the route. The route is: send an email to mohammad@movahedi.ca naming the material and the engagement it relates to. The agreement terms for that engagement are then agreed in writing before the material is provided.
  5. Step 5: If a document does not exist, the reply says so. If the document you named does not exist, the reply says it does not exist. The reply then offers what does exist instead, from the table below, where a related document covers the subject you asked about. No document is created in response to a request and presented as if it already existed.

How this works. A requester names a document and a purpose, and gives a name and an email for the reply. A public document is read directly from its link. An on-request document is sent by email in its current version, with its date. Material that needs an agreement is routed to the agreement step, with the requirement stated. A document that does not exist is stated as not existing, with a related document offered where one covers the subject.

Document Request form

There is no server backend for this form, and no submission is stored on a server. The form does not create an account, does not assign a tracking number, and does not send an email on its own. On submit, the form prepares an email addressed to mohammad@movahedi.ca, with the form fields filled into the email body, and opens your own email program to send it. The request is complete only when you send that email. If no email program opens, the form shows the prepared request text and the address mohammad@movahedi.ca, so you can copy the text and send it manually. No confirmation email is sent automatically by the form. A reply, if any, comes by email from mohammad@movahedi.ca, under the procedure above.

The form collects only the five fields below. It collects no payment details, no passwords, and no other personal information. It does not attach files. A request for a document is a text request only. Closing the page before sending the email loses the prepared request.

State why the document is needed, so the request can be routed correctly, including where an agreement may be required.

Your full name.
